Chronic Disease Management, WorkCover & DVA
Sera Spine & Rehab is registered to see patients under CDM plans, WorkCover, and DVA — at both our Elsternwick and Cheltenham clinics.
Chronic Disease Management (CDM) plans
A CDM plan — sometimes called a GP Management Plan — is a Medicare initiative that allows your GP to refer you for up to 5 allied health sessions per calendar year. It's designed for patients with a chronic condition that has lasted, or is likely to last, 6 months or more.
Please note: Sera Spine & Rehab does not bulk bill CDM patients. A gap fee applies. Our reception team can advise you of the current fee schedule when you book.
How it works
See your GP
Ask your GP if you're eligible for a CDM plan. They'll assess your condition and, if appropriate, prepare a GP Management Plan and Team Care Arrangement.
Get your referral
Your GP will refer you to an allied health provider. Make sure to bring your referral paperwork and Medicare card to your first appointment.
Book with us
Call us or book online. Let us know you're coming in on a CDM plan so we can prepare the right paperwork before your visit.
Start your treatment
Your treating practitioner will assess you and develop a treatment plan tailored to your condition and goals.
Eligible services at Sera Spine & Rehab
Spinal assessment and manual therapy for musculoskeletal conditions including back pain, neck pain, and headaches.
Evidence-based assessment and rehabilitation for injuries, chronic pain, and movement dysfunction.
Soft tissue therapy targeting muscle tension, trigger points, and chronic musculoskeletal pain.
Structured exercise programs to restore strength, mobility, and function for long-term health.
WorkCover & DVA
WorkCover
If you've been injured at work and have an accepted WorkCover claim, we can provide chiropractic, physiotherapy, myotherapy, and exercise rehabilitation services. Please bring your claim number and insurer details to your first appointment. Pre-approval from your insurer may be required before treatment begins.
DVA (Department of Veterans' Affairs)
We welcome DVA Gold and White Card holders. DVA covers a range of allied health services for eligible veterans and war widows/widowers. Please bring your DVA card and any referral documentation to your appointment.
